In response to the increasingly vital correlation between patient travel and proper medical diagnosis, Wilmer recently made an important update to its Patient Sign-in Label form. Labeled as "Out of the Country Recently," the new column appears under the "other questions asked" section of the form.
At check-in, patients sign their name on the next available line on the form while indicating appointment time, who the appointment is with, whether or not they are a new or existing patient, along with any changes to address, telephone or health insurance coverage. The update now asks patients to specify whether or not they have been outside of the country. All of this pertinent data is still kept confidential by either the patient's or receptionist's removal of the protective label depending on the style of form chosen.
